1/18/2024 EnchantaVaughn Approved QMHP-A inquired about Ms. Broadnaxâs thoughts, feelings, and emotions. QMHP-A addressed Mental Health Education with Ms. Broadnax and engaged her in a discussion on the importance of adhering to her medication on a consistent basis to stabilize her mood and attempt to reduce some of those depressive symptoms she encounters. QMHP-A educated Ms. Broadnax on the symptoms of her Major Depressive disorder and how it can negatively impact her relationships with others. QMHP-A continued to encourage and reinforce the importance of identifying the triggers of her stress and depression as well as the importance of verbalizing her thoughts, feelings and emotions rather than isolating herself from others and suppressing her feelings. QMHP-A and Ms. Broadnax reviewed once symptoms of Major Depressive Disorder with respect to how it can impact her physically and socially. QMHP-A assisted Ms. Broadnax with scheduling an appointment with her PCP as well as her psychiatrist due to her communicating with QMHP-A she is trouble sleeping at night. Ms. Broadnax appeared to be receptive to QMHP-Aâs interventions and services that were provided to her during the session. Ms. Broadnax was receptive to implementing relaxation and calming skills to help her decrease some of her anxiety. Ms. Broadnax shared that she does not know how to distinguish between being assertive verses being aggressive. Ms. Broadnax stated, âPeople say that I am unstable and I do dumb stuffâ. Ms. Broadnax shared that she feels her life is useless at times but she is never going to give up. Ms. Broadnax was able to identify that feeling inadequate triggers her depression and her feelings of paranoia triggers her anger. Ms. Broadnax has agreed to begin thinking positive and filling her day with positive things to do so she can help to decrease some of her depressive symptoms. At the end of the session, Ms. Broadnax was able to schedule her appointments with minimal assistance from QMHP-A.
